The 2009 restoration of registration form is available below. Please be aware that;

• You can restore your registration online (conditions apply) – click here
• You can type MOST information into the 2009 restoration of registration forms
• You must PRINT the completed form prior to signing it
• You must RETURN the printed and completed form to the Board by mail at locked Bag 1341, Brisbane QLD 4001 or in person at 8th Floor, Forestry House, 160 Mary St, Brisbane QLD 4000
